2nd Failure At Numbing** Please Advise

Is Articaine the strongest / best LA ???

HI :) everyone ! Please would you kindly help?advise? as this is the first time I have ever posted on a forum!!

Last dental experience 10 weeks ago involved a large amalgam filling to my upper right 6 molar. It took 3 cartridges of anaesthetic to freeze me, and after waiting about 30 mins she managed to!! My mouth was crooked, I could not feel the right side of my face and half my nose was frozen, but it worked.

Now I go to new dentist because front of this filling now hurts me and think there is a little decay she missed. So new dentist could not freeze me either !! After 1.5 cartridges, he adds a little Articaine....NOPE....STILL NO FREEZE !! He swished cold water round the tooth and I even felt that !!! Also UR 6 & 7 seem to be sensitive in the gum area too, BUT SHOW NO APPARANT INFECTION.

Now I am taking 2 sets of antibiotics, just incase...........
PLEASE HELP ME GUYS....AS I AM WANDERING AROUND IN THE DARK HERE.....and my confidence is dropping by the day !!! Hugs, Vanessa
 
Articaine is not the best anesthetic, but it has a really fast onset so it works great. Lidocaine works just as well. Do you have an infection? Is that why you're taking antibiotics? Sometimes its hard to numb teeth that have an infection around it. 1.5 carpules in a healthy individual is nowhere close to maximum recommended dose so there is no need to be worried. I took out 12 teeth in a lady today with using 2 carpules of articaine and 3 carpules of lidocaine. Some people just need more than others. Don't give up!

Your reply is very helpful...........no the dentist was NOT CERTAIN THAT I HAD INFECTION.....it was not on the x ray......but he thought it best to give amoxicillin capsules 250g + metronidazole 200mg for 7 days, just incase there was something residual there, but not showing!

Is that right? Also my nerves in UR6 + UR7 are inflamed from their old large amalgam fillings so I might have to have UR7 extracted at some point.

I don't want root canal on it and it's so far back you won't see.

It's very hard for a layperson, because you just have to trust someone else's expertise, whom you don't really know!! :confused:

DECAN....PLEASE CAN I ASK YOU?? I NEED A SMALLISH REPAIR TO ONE FILLING (THE ONE THEY COULDN'T NUMB!!) PLUS 3 OTHER LARGE DEEP REPLACEMENT OF AMALGAM WITH WHITE FILLINGS.

IS IT SAFE TO GIVE 3+ CARTRIDGES LIDOCAINE EACH TIME FOR EACH TOOTH?? ISN'T THIS A LOT OF ANAESTHETIC IN MY SYSTEM??

THE DENTIST SAYS I HAVE "HYPERSENSITIVE" TEETH AND MAY NEED TO GO SEE A SPECIALIST CONSULTANT & HAVE ALL THE WORK DONE UNDER GA IN HOSPITAL??? DO YOU THINK I WILL HAVE TO IN THE END??

3 MONTHS AGO MY OLD DENTIST EVENTUALLY MANAGED THE FILLING WITH 3 LIDOC AND A 30 MINUTE WAIT FOR IT TO TAKE, SO HALF MY FACE WAS FROZEN AND MY MOUTH WAS CROOKED, I COULD NOT FEEL HALF MY NOSE, BUT IT WORKED...............................:hidesbehindsofa:
 
I'm sorry, but its tough to say if you'll have to go to the hospital or not. The maximum dose of any anesthetic is based on your weight. In the States the maximum recommended dose of 2% lidocaine with 1:100,000 epinephrine is 300 mg. This doesn't mean everyone can get 300 mg worth of anesthetic, because you have to also consider maximum dose of epinephrine. It must be calculated based on your weight. Here we have carpules with 34 mg drug/carpule. To figure out how much lido you can receive, plug your weight in the following formula:

(____ lbs X 2 mg/lb)/34 mg = y carpules max

Now, this is all based on safety guidelines in the US. Depending on where you're from the guidelines may be different. Also, this formula works for 2% lidocaine...if, for example, your dentist chooses to use another anesthetic the max dose may be different.

If I understood correctly your dentist has to work in your two upper right molars. Since they are both innervated by the same nerve he/she may choose to do a nerve block (here, the anesthetic is given next to the nerve so multiple teeth are numbed at the same time) instead of a field block (here, only the tooth being worked on is numbed). So, instead of 3 carpules per teeth, your dentist will use 3 carpules to numb up the nerve which would keep you well under the maximum dose.

I know there was a lot of technical jargon there, but I hope it made sense and I hope it helps! :)

I am in UK so don't know what the guidelines are here............

Sensitive Teeth Which Can't Be Numbed??

AFTER MY DENTIST gave me the LA, he said my gum was numb as I did not feel any pain when he applied pressure. BUT I did feel COLD SENSATION as he swished water around the top of my molar tooth where the tooth meets the gumline!! :cry:

He could not do my filling as I had sensation there. I could definitely feel it.

please, please can someone help me?? WHAT ON EARTH AM I SUPPOSED TO DO NOW?? IT SEEMS I HAVE HYPER SENSITIVE TEETH !!!!!!!!!!!

IS MY ONLY OPTION HOSPITAL AND GA ??? HOW MUCH WILL THIS COST IN UK???? I AM NOT RICH.............PLEASE CAN SOMEONE HELP ME??
I need 4 large fillings :hidesbehindsofa: and I have very old amalgam ! :cry:

There are different injection techniques e.g. Gow Gates/Akinosi etc and stronger brands of LA they can try e.g. septocaine.
Doubt you would need GA but if it were an NHS referral to an NHS hospital, it would be 'free at point of use' in UK, as is the norm.

The truth is that all teeth can be got numb but not all dentists can get all teeth numb all the time. You may need to change dentist or see someone at the hospital using a different method (not necessarily GA).
Saying you are hypersensitive sounds to me like the dentist making excuses for their inability to get you numb. I doubt they have exhausted all the avenues with just local yet but they may think they have.
