I was in the ER in and out for the past few days, my left lower lung lobe collapsed, can't breath, pain where the collapse lung is, doctors do not know why, it was suddenly.

The only thing that was different in my life was 10-12 days ago I have multiple molars extraction, there was ALOT, and I mean ALOT of Lidocaine inject to get me numb, due to gum disease, it was infiltration method (not nerve block), and it took ALOT ALOT of Lidocaine to numb me.

I mean it very very rarely for dental Lidocaine to cause lung collapse right? I read Google it said Lidocaine can caused that but it more like IV Lidocaine like in a surgery setting, like if you have IV Lidocaine drip for few hours.

I'm just over paranoid, can dental Lidocaine cause partial lung collapse?

@gumdisease This may not exactly answer your question, but in case this information is any use, the same thing happened with my spouse, random collapse of the lingula of the lung (left side) we went to the ER too. There was nothing different in their life either, they hadn't had dental work, our doctor thought it might be caused by a virus, and in the ER they didn't say any cause but pointed out my spouse is carrying extra weight. The doctor said they could have a virus that could cause this with no symptoms.

No it can't. Even so, 11 days later, it's broken down and excreted inside a day...

Hi, I have Liver hepatic steatosis, which I'm assuming it still work ok-ish until it progress to cirrhosis (I'm not at this stage yet), it good to know now as I have no teeth left to eat meat anymore, so I will be loosing alot of weight from soft food, so hope my liver will heal.
I didn't know I have fatty liver until my CT CAT scan of my collapse left lung show.

But even so, the Lidocaine toxicity symptoms unlikely to cause lung collapse right? As I was still wake the whole procedure, it wasn't like I be put under general anesthesia.

Maybe next time extraction I will ask for a nerve block as it use less Lidocaine than infiltration, as last time it took alot of Lidocaine to numb my gum disease molars.

@Gordon

Sir, I think I remember the tube of the injection, it say 2% Lidocaine with Epinephrine to tube was 1000mg, and he use it all on my multiples molars. Can you overdose on 1000mg Lidocaine when inject it in gum infiltration?

No, I don't blame the dentist, it was hard to numb me, and there always a risk of adverse effect, but isn't 1000mg Lidocaine is alot?

@Gordon

Hi, I was awake the whole time so I remember, it was 1 cartridge of Lidocaine of 1.7 mL which is 1000mg

And he only use 1 cartridge, I do not recall he use the extra one that was on the table. And I'm an ADULT, not a child, so 1 cartridge of Lidocaine would it give me overdose toxicity?

Not even close. For a 65kg adult you'd be looking at 12-14 2.2ml (European size cartridges) it's complicated a bit by the epinephrine, so in reality nearer 20... possibly more

@Gordon

Thank you Dr.
Next time I get my remaining teeth extract (which one tooth at a time will lead me down that road since I have advanced gum disease), I will put in my form I have a Fatty Liver (as this was diagnosed in the CT scan) when they found my left lung lobe collapse.

Fatty Liver is actually common here in America, I'm obese, like very obese, I eat alot of pork fat all my life, so this definately contribute to my Fatty liver, as I never have a drop of alcohol in my life, just alot of pork belly fat type of food.

My liver deceased in size. So it not function normally like a big healthy liver.

Well now I no longer have molars, I can't eat pork anymore, now is soft soup mashed soft food diet. This should help improve my Fatty live.

Next time I just ask them to becareful for Lidocaine dosage since my Liver is not normal like other people.

Thank you Sir, as you can tell I have anxiety, sorry for keep ask questions.

I do have a ques to ask you Sir.

I read an Medical journal said:

Fatty Liver Is an Independent Risk Factor for Delayed Recovery from Anesthesia.

The thing is there people who have Fatty Liver and not know, unless they have a CT CAT scan of their abdomen and chest like me.

And I'm sure Fatty Liver people will need teeth extraction or dental work somewhere down their life.

I know this has to be mention to my dentist and will do. But can I still have a Fatty Liver and still use Lidocaine just make sure the dosage is not too much. I rather use Lidocaine than put under, because General Anesthesia is not a good idea for those who has Liver disease.

But then General Anesthesia is different from local Lidocaine though.

I will start with mentioning I’m not a dentist. If I look at the Canadian Dental Association, I found some recommendations, not sure if latest, saying they recommend to only treat one quadrant at a time for people with severe liver disease. For others, it’s mainly to stay below the maximum limit.
