I wondered if I could ask if you see anything concerning with the two teeth behind the two root canal teeth (molars)? I know #15 my last molar is not fully visible in these photos but I am wondering if you see one that looks like it needs a root canal? I am being told maybe but I am not quite sure what they are seeing? Thank you in advance.

Dr. Gordon - Just wanted to recap on my history. (two root canal teeth above are #12 and #13) Pain in my upper left mouth began 2019. My endo RCT #12 and after I was still in pain we moved to #13. Pain never subsided. Now I am having new pain with hot foods and constant aching which feels like it's in #12 but mostly #13. The molar next to them (14) is cracked with a huge filling. That one never tested positive on any of the root canal tests, actually no teeth gave clear indications. My dentist sent this over to the endo's office and they say 15 needs a root canal. 15 has no fillings. I am debating just pulling the teeth at this point. I just feel defeated. He made the diagnosis on 15 (very last molar in pic) based on these x-rays. I wanted to see if you happen to see it? I am starting to lose trust and am going to get a second opinion.

I had seen an orofacial pain specialist in the mists of all of this last year who did a cone beam scan. He felt the two teeth never should have had root canals and he did not think this pain was a dental issue. He wants me to get an MRI. I have not done that yet as it really feels sensitive to heat. I have developed extreme dental anxiety from all of this. I am at a loss. Thank you and I am so sorry this is so long winded.
 
Hi Acheygirl,

I am not surprised you are at the end of your tether. It must be so frustrating when the local anaesthetic wears off to find you are still in pain. The nature of nerve pain in teeth is such that it can be very hard to pinpoint exactly where it is coming from, and other non-dental related conditions, can also mimic it just to add to the confusion.
To be honest, I cannot see anything on the x-rays that would enable me to diagnose enough to prescribe further treatment. There is simply not enough of tooth 15 visible to deduce anything.
Sometimes, with time, the pain locates onto the offending tooth. An MRI may be helpful to exclude other non-dental causes. It may also be worth asking your dentist if he or she thinks you grind or clench your teeth as this can sometimes be a factor.

Dr. Hirst thank you so much! If I can ask one more question, do the crowns on there appear ok margin wise? I am just grasping at straws. I do have a night guard but i've been waking up with some pain so I have not been good wearing it.
 
The margins look fine on these films. They wouldn't cause pain anyway even if they weren't, sorry.
