Dull pain (mystery)

Hello, the circled tooth has been aching on and off for months .. lately it’s been worse than ever and I’m wondering if it looks like there’s anything wrong with it based on the X-ray attached. It’s a spontaneous pain and is not triggered by hot cold or chewing. Any advice would be so much appreciated. Thanks in advance
