Extraction and Alveoplasty with just Novacaine. No sedation due to COPD and GERD.

I have to have six lower teeth extracted, have an alveoplasty, and then put in a lower denture. The oral surgeon told me he was not able to sedate me or use nitrous because I have mild/moderate emphysema. I also have GERD/ chronic gastitis and have problems lying supine. He told me the only thing he could use is Novocaine. I've spent thousands on my teeth over the years, and Novacaine does not help me at all. I also have a horrible fear of needles. I'm nervous that the Novacaine dripping down my esophagus will make me regurgitate or vomit. Do you think there's a way the dentist can perform this procedure with me sitting up in the chair? Is the other option having it done in the hospital?
 
Novocaine hasn't been used since about 1945, I'm not surprised it's not been good for you :-) (Sorry, that's a pet hate of mine!).

No reason why you can't have the work done sitting upright, it's easy enough for the dentist to accommodate this for you, especially for lower extractions. I don't see any need to go to hospital for it.

Local anaesthetics have come a very long way since Novocaine, you should find them much more effective. It also should emphatically NOT be dripping down your oesophagus.
