Granuloma - tooth extraction

And I am back again, uff!!
Two years ago I had a root canal on a lower molar that has been very traumatic.
It lasted several hours, dentist was super tired, and I had horrible pain the night after.
The tooth never completely stopped hurting, was showing some inflammation at x-ray, so one year later the dentist retreated one canal.

After 10 months I still bave pain.
I asked a second opinion and got a scan today which showed a granuloma (? Gray area at the apex).
The dentist told me treating the tooth for the third time isn't likely to solve it.
Being a back molar he told me the apicoectomy would be difficult and painful.

He suggests an extraction and putting an implant.

He gave me a 5 days course of antibiotics as I have some pain, and extraction will be in 3 weeks.

This is my fourth implant and I am <45 yo :(

The other three are upper teeth though.
I am scared of the lower tooth extraction (and usual alveolitis). Thanks God I quit smoking and vaping, but still scared).

I am unhappy of the antibiotics also. The granuloma has been there for 2 years so I hope it will stay quiet other 3 weeks.

A part of me wants to ignore it, as I lived with the discomfort for two years waiting if it would solve (advised by the dentist).
But I know it's now very unlikely and I risk a cyst or abscess.

Sigh :(

I don't know what I am asking, I need maybe to know if it's a right choice.

Completely agree with the dentist on these points.

Put your Vulcan's ears on and it's the most logical choice, much higher chance of success, less invasive surgery and long term probably not much more expensive. :-)

Root canal is not fun but I remember apicoectomy wasn't a very nice recovery either.
I just dislike another extraction and having another implant uff but glad to know it's sadly the right path ahead
