I have 2 molars (first molar and second molar) next to each others that are both loose mobile due to gum disease. Dentist did ask if I want to remove both at once since it same quadrant, I chicken out so I said one first and I come back later for the other one.

I just had my first molar extract 4 days ago so the extraction gum is not heal yet. My adjacent tooth which the second molar now is super loose, so this one has to go out soon.

So how long do I wait till the extraction site of the first molar heal before I come back to dentist to extract the second molar that still left in there?

Would couple weeks be good? Or a month? I guess I wait for the extraction hole to close then it safe to extract the adjacent one?

Thank you.

It doesn't really matter, whenever you feel up to it or the dentist can fit you in. Healing is pretty quick with periodontally involved teeth.

@Gordon

Thank you Sir dentist, I called and the dentist is pack schedule, so fastest I can get in is 10 more days, the tooth is loose like loose loose but I think it can wait 10 days as it is bottom Molar and adult molar can't just fall out on it own, I hope not.

Sir, why is healing is quicker with peridoontal involved teeth? It is because it loose so it easier to come out so there not much trauma to it while try to get it out as if it was a normal person teeth with no peridontal disease? I mean I would imagine a normal person with no peridontal disease their tooth is vary hard and not mobile level III like mine.

I be honest Sir, all the teeth that I have gotten out so far they all due to my advanced gum disease and it loose, and bone loss, and my gum just cannot hold the tooth together anymore.
No cavities at all, because the calculus prevent cavities but then the calculus is what cause the inflammation of peridontal since the bacteria attack my gum and eating away my bones.

And this will be my tooth #3 extract in one month period of time, I won't overdose right?
November alone I have 2 teeth extraction, and coming up next week in Dec I woll have tooth #3 extract.

But since it space out time like two weeks apart each, I won't overdose on Lidocaine stuff with Epinephrine right?

Thank you Sir.

No. it gets broken down in your liver in a few hours.

Because there isn't much bone left round the roots, bone heals more slowly than soft tissues. Less bone, faster healing.

Thank you Sir @Gordon, one last question. My extraction is schedule next week, I think I might be on my period/menstrual, but can I just go on with the extraction? as it Level III mobile, it moving in all directions left right, up down, front back, it the dentist told me that it is Level III mobile, it cannot be save.

It so bad Sir, not just bone loss (which can't hold my tooth in place) but also my gum is so swollen that it pulling away from the tooth that the gum can't hold the tooth in place neither.

I want to get it out before Christmas just in case the dentist go on Christmas vacation.

If the tooth is this mobile, can it be done while on menstrual? I do have high pain tollerance, I just hope I won't get Nerve damage, will be asking for Infitration again which he will deposit Lidocain around my tooth roots.

The dentist was against doing Infiltration on bottom Molars since he said bottom jaw bone are hard. But with my condition on the Xray he see the bone loss and he said ok, Infiltration can work. And he said it will be easy pull. And it was easy pull due to it mobile.

I'm 40 Sir and I never take care my teeth and now I am paying the consequences for it, it will be Denture soon for me. Plus my mother have very bad teeth she loss all her at 40, so I got the genes from my mom, I'm 40 now and yah I'm just like her, denture for me.

oh and the dentist told me, gum disease is true that due to I neglect my teeth hygeine, but it also generics too, like I got the genes from my mom.

There's no reason why you being on your period would stop you getting the tooth out.

So Dr. @Gorgon

I have a question, I just came back from extract the tooth, I got in earlier as someone cancel.

Doctor was busy today, alot of patients.

So the doctor do Infiltration, numb me and leave me for 10 minutes as he said he has a child tooth need to be pull next room so he leave me for 10 min for Lidocane Infiltration to numb me. Should I worry that he was gone for 10 min? I mean it just Lidocaine right?

Then 10 min he came back and he said I need MORE Lidocaine, for some reason today I was not numb as fast as last time I think it due to I eat ALOT the night before, I know I couldn't eat for 24 hours after extraction so I just stuff myself with food last night like I was eating Thanksgiving meal.

But he inject more Lidocaine and extract the tooth. The tooth is out.

However as I was sitting in the waiting room waiting for my ride, I notice my oxygen level drop a bit for a minute or so and then it went back up, I think it due to the Epinephrine.
My oxygen level is fine now.

So I guess I'm okay right?

I just worry he inject me with Lidocaine and then leave the room for 10 min go next door extract a child tooth and back for me.

It just anxiety, same Lidocaine 2% with Epinephrine. But for some reason the Lidocaine wear out early today (I'm not as numb today as last time), maybe it just my hormones today. Or all those food I ate the night before.

oh and because he inject extra Lidocaine, the Lidocaine liquid was on my toungue near my throat and I accidcently swallow some, just a tiny amount that come out of the needle. I will be OK right?

And I read the cartridge today, Lidocaine 2% 1:1000 Epinephrine.
He use 2 cartridge, and I'm an adult, can I overdose on Lidocaine 2% with two cartridge?

No.

It's not absorbed through the skin, which is why we need to inject it... so yeah, you'll be fine

What were you worried about?

However as I was sitting in the waiting room waiting for my ride, I notice my oxygen level drop a bit for a minute or so and then it went back up, I think it due to the Epinephrine.
So you were wearing a pulse oximeter in the waiting room?
SaO2 naturally varies over a few percent in a healthy adult over time, it's quite normal.

I had my extraction yesterday afternoon and this morning I wake up my menstrual come, so maybe that was why it harder to numb me this time.
Last time after the extraction I was numb for 4 hours straight, this time after extraction I was numb for 30 min and it wore out and my extraction site started throbbing and hurt, so I know the Lidocaine worn out.

I am sure he does infiltration on me, as I can feel the needle pinch in my gum. He has to give me 2 round of Lidocaine, first round after he inject me then left 10 min to extract a child tooth.
Then he back and said I need more Lidocain so he add some more, then he extract my tooth.

Anyways, this is my worry.

I had a pulse oximeter the one where you put on your finger, I never bring a pulse oximeter with me but this time I did (as I read in Google that Lidocaine can cause oxygen drop), and while I sit and wait for Uber (didn't want to drive on day of extraction), I put the pulse oximeter I notice my oxygen were dropping. At first didn't worry thought the oximeter just being whack.

Then my Uber arrived I just have to rush out to Uber. Then while I was on Uber 5 minutes later I measure my oxygen again, so it within 30 minutes window of since the first round of dentist inject my gum to the time I get on Uber. So the reaction might be delay reaction.

This time Lord behold, the oximeter was flashing both heart rate and oxygen rate, my heart rate was showing 44 and my oxygen showing 76, BOTH were FLASHING like crazy (meaning it in danger zone).
.....
I got scare, so I took it out and put it on the other finger, show the same number.!!
.....
Then I got really scare I told the Uber to drive me straight to the hospital. Uber said he needs to get off freeway first.
I did NOT feel dizzy or faint or blackout, I still have concious to talk to Uber driver to change route to hospital.
.....
BUT pulse oximeter show my heart rate of 44, and oxygen of 76, then few min later, like two minutes later, then number go back up jump straight back to 98 oxygen and mid-80s heart rate.

Then I told Uber driver to took me home please, if it go back down I call ambulance at home.

And never been down since, I have been back to normal level, 98 oxygen.

This is so ODD. As I had Lidocaine with Epi before, this is my six times extract teeth in three months of time and it all same Lidocaine.

I don't know if oximeter glitch. But whatever it was to be safe next time I will bring an oximeter with me and sit in the dentist office waiting room chair for 30 minutes just to be safe before I go home.

And if it matter, I have a fatty liver due to being obesity, there no scars on my liver, it just fatty liver due to I eat alot of bad fat in my diet, my liver still function blood test show it functioning.
Doctor said I need to loose weight and my fatty liver will heal.
But fatty liver people have extraction all the times,
and fatty liver is not cirrhosis, so my liver should be able to get rid of Lidocaine.

I will tell the dentist next time about my oxygen drop incident before I have another extraction, so he will monitor me, just to be safe I just sit in the waiting room for 30 min before I call Uber home.

I will use Lidocaine again, as this is what I use all the times and every time during extraction.
Will not use another local anesthesia, as a new one I'm even more scare of reaction. I rather deal with Lidocaine.

It just so odd, maybe it just a one time off.
