I have posted earlier. But really need some real help here. Dont delete this post.I have lower jaw pain under ear, ear pain, neck pain for 5 months now(since may). Didn't injure anywhere. This started after I chew some meal more than usual. Its chicken and fish. Not bone or any hard thing. I felt that my upper and lower molars were tight after the meal.especially the lower molar. I flossed it. Next day I heard a pop in the jaw and pain, swelling started that evening. It's the same pain under ear in the lower Jaw till today. Nothing helps. Tried antibiotics, steroid pack, muscle relaxers, ibuprofen.
I have been to ENT in June and there is no infection in sinus and ear. Went to dentist after that. He told me that I have a tooth fracture on lower first molar and that is the reason for my lower jaw pain. Also, told me to extract upper wisdom tooth.
I extracted upper wisdom tooth on July. Still the same symptoms. Even few more symptoms added. Like, i started having ringing in my ear often and soft humming music sound in ear while lying down when ac is on. If I turn off, sound will go off. Sore pain on top of head above the ear and hurts to touch. Neck feel so tight and stiff. Constant feeling of something stuck in throat. I have mild swelling in the lower jaw ( since may) and on side of the neck ( since July). I have a small pea size lump on the neck. My PCP ordered a neck USG and no abnormalities found.
I went back to dentist. He says it night be from the tooth fracture on lower first molar. Told me to extract or do RCT. I went to Endo in June and he couldn't find any tooth fracture and that tooth has bite and cold sensitivity. So he told me to do RCT. Meanwhile, I saw a tmj specialist and he said that muscle tightness and told me to try PT. Did PT for 3 weeks. It makes my pain worse. It includes dry needling, intra oral massage.
I waited till September, the same symptoms. No change. It's constant burning pain in jaw and neck. I went back to Endo and had rct. Since my dentist said we can spot the fracture if we open the tooth. But the Endo couldn't find any fracture on that tooth. He finished rct. It's been 4 weeks after rct, I still have the same symptoms. So, my dentist said it could be tmjd or eagle syndrome. But he also believe that suspected tooth fracture on that tooth could be the culprit for my pain. I went back to Endo, he don't want to open the tooth again. He told me to try temporary crown on it and see if it helps. He said he could see some fracture lines on that tooth and that might expand if I delay the crown. If pain persists, he said we will reopen the tooth later.
Meanwhile, my dentist sent me to a tmjd specialist. He did pan, cbct, tomo x-rays. He did a thorough exam. He said it looks like muscle tightness, myofacial pain and jaw joint issues. He said it's like chicken and egg. Don't know which one cause the other. He suggested a treatment plan of day and night appliance(guard) which is very expensive and not covered under insurance. He also told be to do PT along with that. He couldn't find any calcification in cbct regarding eagles syndrome.
My PCP felt that this could be trigeminal neuralgia and referred me to a neurologist. I asked tmj specialist is this is a neuralgia. But he said, of ours neuralgia it will increase slowly. You will not feel pain immediately on one day. It will be a gradual pain.
But he is not sure why I have neck pain. Burning in neck pain. He said, we can try the treatment for 4 months. If it doesn't go as planned,I will order mri head and neck for further diagnosis. This was his answer.
I am summing up my symptoms.
-constant burning pain in lower jaw under ear since may.
- mild swelling in lower jaw since may.
- ear pain and feel clogged ( since may)
- ringing in ear often and some soft humming music sound in ear while lying down when ac is on. Since august
- sore pain on top of head above ear on one spot. It hurts to touch. Since July and I felt this after wisdom tooth extraction.
- neck is so tight and tight. Mild swelling in neck and one small lump. Since July.
- burning pain in neck since July
- constant feel of something stuck in throat since August.
- sore pain on base of head above my back.
Attaching the images and notes. Alps adding my cbct images which I have taken for my lower molar tooth. My dentist took this and he said he suspect a fracture on my tooth. But the Endodontist didn't take any cbct or see this cbct. He said we can't see anything on cbct and didn't even get my cd or take a new one.He saw only xrays. Whether can we see fracture on cbct?
My dentist suspect that tooth fracture could be the reason for my symptoms and I went for RCT irrespective of no pain on that tooth. I was looking for some relief and did this rct.
Endo couldn't spot any fracture during the procedure. But my dentist still suspect that tooth could be there culprit for my pain and symptoms. He didn't do the temporary crown yet. It's been 4 weeks after rct.
Meanwhile, I have contacted a dentist from my country and he suggested to get the reporting done for cbct images. He sent to a radiologist there and I got the report for tooth cbct scan today.
In that tooth cbct scan, he has mentioned like suspected distal root fracture on that tooth. Does it need extraction or rct will help? please help.
