Jaw pain and swelling for more than 4 weeks

Yes PT helps a lot with TMJD. I have no knowledge of eagle syndrome, sorry.

I am still wondering if anything on that tooth that is still causing the pain after root canal. Is that possible to miss the root crack or fracture during the root canal. Whether they could cause these pain and symptoms or tmjd?

Whether tmjd cause these symptoms ? Have you seen in your year did experience? Is it like from crack/fracture or tmjd?

I am also getting a night guard from my dentist. I didnt get the temporary crown done on that tooth yet.

It's pretty unlikely to miss a crack/fracture during a root canal, the trouble in your case is both could cause the symptoms you're having. The easiest one to eliminate is the TMJ with a cheap night guard, which sounds like what's happening?

I couldn't get the cheap night guard which my dentist said. It's 150$ for aqualiser. It's a temporary one, so I didn't get it done. I have asked for mouth guard which costs 650$ out of pocket. But this is way cheaper when compared to tmj specialist splint which is 5500$ treatment plan.

I don't know what do you mean by cheap night guard?

And I still feel tender pain on the gum area and tooth which I had rct. I am babying it and doesn't use that side at all to chew. Just brushing with soft bristle and cleaning sometimes with hand if I see something stuck on the gum area.it feels tender to touch that tooth and gum. Hope it's normal and am still taking pain medicine. I am yet to get core buildup and crown on that tooth.

You can buy a sports mouthguard from the likes of Walmart for next to nothing. If it's TMJ due to clenching/grinding then wearing that for a few hours per day will probably be some help. If it helps, then you're closer to solving the mystery :)

I had a follow up with my dentist. I told him that I have the same pain in the jaw and neck. Still the same symptoms. He doesn't want to do core build up and crown until the Endo cleared there is no root fracture in that tooth. I told him that I asked Endo about it and he said he couldn't see during the procedure.
But my dentist said he don't want to risk and do more damage to the tooth if root fracture is present. He wanted me to get confirmation from Endo that tooth is not the culprit of my pain and symptoms. I asked Endo office and they said if needed I can come in for a check or they will send the rct report to my dentist.

Will the Endo will be able to check now to see if any root fracture is present? After 2 weeks of root canal? I don't know why this is not so visible and not easy to diagnose though.

Since yesterday, I have pain on that tooth and gum area. It's like sore pain and sometimes sharp pain. I am not using that side at all for chewing. Am not even trying to touch /hit the upper and lower teeth together that side. I am not flossing too. Just brushing with soft brush. Didn't use that tooth at all for chewing.

So, I am confused if still this tooth could be the culprit for my lower jaw pain under ear. Why it's paining in the tooth after root canal. I am planning to go for a follow-up with the Endo. Hope he can check the root fracture. Is there a chance to identify the fracture street the root canal doctor ?

I am trying an over the counter mouth guard for past few days. It doesn't make any difference yet and have to wait and see if it helps me..

Just an update and would like to know your inputs on this.

My dentist told me that it could be either tmjd or eagles syndrome. My PCP told me to rule out trigeminal neuralgia as well. My dentist referred me to a specialist. He is a tmj dentist. Not an oral surgeon though.

I have consulted him and he took an tomography and pan images. He told me that I don't have eagles syndrome. He also said I don't have neuralgia as my symptoms didn't increase gradually. He said it will increase gradually in the case of neuralgia. Mine is all of a sudden pain one day and still the same since may.

He said it's myofacial pain and issue in the jaw joint. The muscles are tightened up and pulling up the jaw joint and causing this burning pain. But he is not sure why I have pain in the side of the neck with small lump and mild swelling. He said to try his treatment for few months and if it doesn't improve, he told me to see someone and get mri head and neck that time.

His treatment plan was day time for 4 months and night time appliance for years. It takes around 6 months to finish the treatment. Costs around 4.5k and not covered under insurance. None of the tmj specialist in my city is not covered under insurance.

He said it might help with the muscle tightness and jaw joint issue. He didn't see any disc dislocation.
He also told me to do physical therapy along with this to improve the muscle stretching and tightening.

I don't know if I can proceed with his treatment or give it sometime by trying physical therapy alone. Anyone had any experience of trying PT for your jaw and neck pain? Please share your inputs.
Attaching the images here. Please let me know your thoughts. The treatment is little expensive and not covered under insurance. So I am just thinking how to proceed further.

I have lower jaw pain under ear, ear pain and neck pain for 5 months now. Went to ENT in June and he did nasopharyngoscopy to check my sinus if it's causing the jaw pain. No infection in ear and sinus. He ordered me a ct sinus scan. I didn't do that yet. I went to the dentist after that. Then he suspected few tooth issues that's causing the pain. I did those treatment&still same symptoms.My symptoms got worse.I started hearing ringing sound in that ear. Hearing some soft humming music sound in ear when lying down when ac or fan is on. It will be there for few mins and disappear. And I have sore pain on one spot of the head above the ear and it hurts to touch and feel some mild pressure. My neck ,throat is so tight and stiff. small lump in neck. Constant feel of something stuck in throat. My dentist now telling me to see tmj specialist to check if it's TMJ or eagles syndrome.Went to tmj specialist and he said it's myofacial pain, muscle tightness and jaw joint issue.Any issues in joint? He also told me to try PT along with his appliance for day and night. He said I can try appliance for few months. If it doesn't improve, I can check my neck pain further with an mri of head and neck. I don't know how to read or understand these images. How to know if issue in my name joint. The treatment which he told was very expensive. I wanted to understand what the root cause before going for expensive treatment.

I have posted earlier. But really need some real help here. Dont delete this post.I have lower jaw pain under ear, ear pain, neck pain for 5 months now(since may). Didn't injure anywhere. This started after I chew some meal more than usual. Its chicken and fish. Not bone or any hard thing. I felt that my upper and lower molars were tight after the meal.especially the lower molar. I flossed it. Next day I heard a pop in the jaw and pain, swelling started that evening. It's the same pain under ear in the lower Jaw till today. Nothing helps. Tried antibiotics, steroid pack, muscle relaxers, ibuprofen.

I have been to ENT in June and there is no infection in sinus and ear. Went to dentist after that. He told me that I have a tooth fracture on lower first molar and that is the reason for my lower jaw pain. Also, told me to extract upper wisdom tooth.
I extracted upper wisdom tooth on July. Still the same symptoms. Even few more symptoms added. Like, i started having ringing in my ear often and soft humming music sound in ear while lying down when ac is on. If I turn off, sound will go off. Sore pain on top of head above the ear and hurts to touch. Neck feel so tight and stiff. Constant feeling of something stuck in throat. I have mild swelling in the lower jaw ( since may) and on side of the neck ( since July). I have a small pea size lump on the neck. My PCP ordered a neck USG and no abnormalities found.

I went back to dentist. He says it night be from the tooth fracture on lower first molar. Told me to extract or do RCT. I went to Endo in June and he couldn't find any tooth fracture and that tooth has bite and cold sensitivity. So he told me to do RCT. Meanwhile, I saw a tmj specialist and he said that muscle tightness and told me to try PT. Did PT for 3 weeks. It makes my pain worse. It includes dry needling, intra oral massage.

I waited till September, the same symptoms. No change. It's constant burning pain in jaw and neck. I went back to Endo and had rct. Since my dentist said we can spot the fracture if we open the tooth. But the Endo couldn't find any fracture on that tooth. He finished rct. It's been 4 weeks after rct, I still have the same symptoms. So, my dentist said it could be tmjd or eagle syndrome. But he also believe that suspected tooth fracture on that tooth could be the culprit for my pain. I went back to Endo, he don't want to open the tooth again. He told me to try temporary crown on it and see if it helps. He said he could see some fracture lines on that tooth and that might expand if I delay the crown. If pain persists, he said we will reopen the tooth later.

Meanwhile, my dentist sent me to a tmjd specialist. He did pan, cbct, tomo x-rays. He did a thorough exam. He said it looks like muscle tightness, myofacial pain and jaw joint issues. He said it's like chicken and egg. Don't know which one cause the other. He suggested a treatment plan of day and night appliance(guard) which is very expensive and not covered under insurance. He also told be to do PT along with that. He couldn't find any calcification in cbct regarding eagles syndrome.

My PCP felt that this could be trigeminal neuralgia and referred me to a neurologist. I asked tmj specialist is this is a neuralgia. But he said, of ours neuralgia it will increase slowly. You will not feel pain immediately on one day. It will be a gradual pain.
But he is not sure why I have neck pain. Burning in neck pain. He said, we can try the treatment for 4 months. If it doesn't go as planned,I will order mri head and neck for further diagnosis. This was his answer.

I am summing up my symptoms.
-constant burning pain in lower jaw under ear since may.
- mild swelling in lower jaw since may.
- ear pain and feel clogged ( since may)
- ringing in ear often and some soft humming music sound in ear while lying down when ac is on. Since august
- sore pain on top of head above ear on one spot. It hurts to touch. Since July and I felt this after wisdom tooth extraction.
- neck is so tight and tight. Mild swelling in neck and one small lump. Since July.
- burning pain in neck since July
- constant feel of something stuck in throat since August.
- sore pain on base of head above my back.

Attaching the images and notes. Alps adding my cbct images which I have taken for my lower molar tooth. My dentist took this and he said he suspect a fracture on my tooth. But the Endodontist didn't take any cbct or see this cbct. He said we can't see anything on cbct and didn't even get my cd or take a new one.He saw only xrays. Whether can we see fracture on cbct?

My dentist suspect that tooth fracture could be the reason for my symptoms and I went for RCT irrespective of no pain on that tooth. I was looking for some relief and did this rct.
Endo couldn't spot any fracture during the procedure. But my dentist still suspect that tooth could be there culprit for my pain and symptoms. He didn't do the temporary crown yet. It's been 4 weeks after rct.

Meanwhile, I have contacted a dentist from my country and he suggested to get the reporting done for cbct images. He sent to a radiologist there and I got the report for tooth cbct scan today.

In that tooth cbct scan, he has mentioned like suspected distal root fracture on that tooth. Does it need extraction or rct will help? please help.


please let me know your thoughts and inputs doctor @Gordon, @comfortdentist . please help sir.

I have lower jaw under ear and neck pain for 5 months now. Went to tmj specialist and took CbcT scan. I have been told it's jaw joint issue, muscle tightness. Given the day and night guard.
Attaching the report. Anyone tried splint and seen improvement? How to get back to normal if we have wear or joint damage?

Saw a tmj specialist and he said it's like jaw joint issues,muscle tightness. I went back to dentist to get the crown done. But before I send the report to him, he was not sure about the fracture and told me to try root canal and see. He didn't spot any fracture earlier with cbct. I did root canal. Since he saw my report, he is suspecting a fracture. Does crown help? Please help needed. Please don't delete the post. Does crown help? No pain or sensitivity on that tooth. It gives little pain while tapping that tooth. My pain is in jaw exactly under ear and neck.

I have constant burning pain in the lower jaw exactly under the ear and neck since the month of May. I have been to many dentists, doctors(TMJ specialist, ent, neurologist) and no definite answer yet. Dentists here please help me. Kindly share your thoughts. I would really appreciate it. History:
I have right lower jaw burning pain under the ear and side of the neck right side since may. It started one day when I was chewing food more than usual and immediately after the meal the pain in the jaw started with mild swelling.

I went to the dentist to check that out. He took CBCT scan . He said, x-rays are normal but he suspect a tooth root fracture(distal) in lower first molar and did bite test. I feel little tender and sensitivity to bite on one spot on that tooth. So he said, he suspect a tooth fracture in the lower first molar #30 and that is the reason for my lower jaw pain under ear and neck.

He took CbcT scan and said he is not very certain and suspecting a fracture. I went to Endodontist and he couldn't see any fracture and did cold, bite test. My molar tooth was sensitive to cold spray and to bite on one spot on that tooth. So the Endo said , I needed root canal.

I had root canal in September and still the same symptoms. My dentist suspect there is a fracture and want me to extract it and immediately implant. Will the crown help in my case?

I don't know if I need to extract the tooth ? I have taken cbct scan at dentist office. But my Endo didn't take a look at this scan. Dentist saw it and said he suspect a tooth root fracture in lower first molar and want me to extract. I got the crown this week and that tooth , gum are very sore since crown is placed.

I had temporary crown before this and sore pain started since the crown and core buildup placed. My dentist still suspect there is a possibility of root fracture and asked me to wait for 2 to 3 weeks, if pain doesn't improve , he want me to extract it. I called the Endodontist back over the phone and asked her about it. He said, he don't see anything related to fracture in clinical findings and he don't want me to extract it. He didn't view my CBCT scan as he didn't have appropriate software to open the cbct images. He told me that he won't extract the tooth only if cbct shows the fracture without other clinical findings. But my dentist saying the opposite. He said this root fracture will not show any symptoms and only cbct will show a little. He is not very certain though and want me to try extraction and see if it helps.

My physician said, it could be tmjd or some neuralgic pain too. So I don't know what's the reason for my pain.I saw tmj specialist and neurologist. Constant burning pain in the lower jaw under ear and side of the neck.

Anyone here know how to read cbct images? Please help if you know.

My dentist want me to extract this tooth sooner. I just did permanent crown on this tooth last week and same symptoms still. Tooth and gum are very sore still. Sometimes gives a sharp pinch like pain. I am not chewing on that side at all. Using the opposite side only. Does this sore and sharp pain after crown indicates a fractured root? Please help. Do I need to go for extraction? I don't want to extract the tooth without knowing if that's the reason for my pain. Please help.
I know this is a long post. Please don't delete. I want to give the history as much detailed as possible. Thanks for reading.
