Nervous about molar

Hey Everyone-So I have advanced periodontal disease. I have a partial on the bottom teeth and have 8 of my own teeth. I go every 3 months for maintenance cleaning. I try to do the best I can to take care of my remaining teeth. My back gum around molar bleeds every so often. I thought my periodontal disease was under control! Any advice would be welcomed!
