This post might seem silly because I had an upper molar extraction 3 years ago that went totally fine… I felt absolutely nothing including the lidocaine shot (which I was most nervous about).
I have a bottom molar on the left side that I am getting extracted in a month. I have heard that the nerve block for the bottom teeth can be painful. I really REALLY don’t want to feel the needle (I have a needle phobia). Is it always painful to get the shot for the bottom molars? I am also nervous about my heart racing from the epi because I get heart palpitations and have mitral valve prolapse.

Not necessarily, a bit of surface anaesthetic and some patience by the dentist and it shouldn't be too bad.

It's only likely to be an issue if the local is accidentally delivered into a vein, again, shouldn't happen. You can ask for non-epi local though, there are plenty available and they work fine.
