Nervous of tooth extraction -- can this tooth to be saved?

Lower right-side teeth as shown in the picture.

Very painful (seems from the nerve inside the tooth) since the Christmas period. No pain at all before.

The pain is intermediate (not consistent), but if it comes, can wake up from sleep.

Currently taking amoxicillin for the infection.

My question is: do I have to extract the second last tooth or can it be saved using method such as root canal treatment?

Need to see it to be sure but it looks in a bit of a mess. Looks like the 3rd molar has damaged the crown quite badly. It might be saveable but it really doesn't look like it.
