Pain three months after extraction

Hi!!

I had my upper first molar extracted ten weeks ago. It had a fractured root and a fistula that the doctor only discovered once he extracted it because it did not show on the X-rays. We only had seen the fracture.

I was in pain for months not only on the actual tooth but mostly on the gum inside the mouth at the top of the tooth when I pressed it, when I smiled etc. it was like I felt it where the root was located inside the socket. I feel it is when the cheek bone starts, or the sinus.

it took over a month after the extraction for the pain to be better but after 4 weeks I was fine. Nevertheless it is two days now that Ithe pain in the specific spot came back. In the extracted tooth where the roots uses to end.

I am really scared, I can’t go to a doctor with the measures and I can’t understand why it is back. I am thinking it couldnot be neuropathic as they kept saying before finding the fracture since I was ok for one month now. Could it be that the doctor did not clean the socket well? Could the fistula form again after a month on it’s one??

I don’t know if it makes any difference but during the extraction and even after three injections when the doctor cleaned the socket I could feel pain at the same spot dee inside the socket I assume where the infection was.

Fistula isn't by itself a painful thing, basically a fistula happens when the infection had gone on for a long time and your body had made a more permanent pathway for the infection to track out, so there's no need to worry about it, it's a totally benign thing. Think of it like a very small pipeline.

The socket doesn't need to be cleaned out after an extraction.

Most likely explanation is that there may be some bone remodelling going on which can be uncomfortable or a boney spicule is trying to come through.
 
I will second what Gordon says, from personal experience. I had my front tooth extracted after a long infection, where I had bone loss. The pain would come and go for awhile, even though x rays would show great healing and bone growth. I had it extracted last October, and now have an implant and crown and still have some tenderness and occasional pain where the tip of the root was. My oral surgeon said it was probably the bone growth and nerves that are still “off” a little. It could take a long time to heal.

I think it is worth checking with an oral surgeon or an NET specialist. 10 weeks is long enough.
 
Thank you for your replies!!! I thought it is esential when there is an infection to clean the socket once the tooth is extracted to remove the infection..

No, it's not generally necessary.

I visited the oral surgeon and he order a CT Scan and i got the results today.. I does not say anything on the paper as stage to be found but I would be grateful if I could have you opinion on it.

The doctors was looking for any problems in the teeth next the extraction.. Still in pain on the first root canaled premolar. Could the be a problem in another tooth that we have not found as yet?

I am realy so sad over this one year of pain, so many expenses, a tooth out and still in pain..

I visited a new dentist today.. he says he can see a cyst on the extracted tooth site. After ten specialists and 2 cts is it possible that they did not see it??

Gordon does this look like a cyst? It a picture form the panoramic. Can there be a a cyst on an extracted tooth??

Nope.
I can see that the root filling looks lit it is a bit short on the premolar there though and there may be a slight apical area present. Got any better films of it?

No I only have a more detailed one from the scan for the second premolar.

One dentist told me that the first premolar looks like there is cavitation under the sealing and that might cause the pain. Can you see that?

I'm afraid that I can't really diagnose anything from these kinds of images, I have no training in their use. Try PM'ing Comfortdentist, he's more up to date on them than me.
