Possible Trigeminal Nerve injury during upper molar root canal, nervous!

Hello all, I am new here but I am grateful that this community exists, hopefully my question can help other people that are trying to figure out what has happened to them, as well as help me figure this out myself.

This past monday (7/3/17) I went to my local endodontist for a root canal on my second right upper molar. She began and administered the anesthesia in two spots, in between my cheek and the molar (PSA nerve block) And on the other side of the molar. I don't remember feeling any type of electric shock thats typical with a direct nerve hit, i didn't feel much but the needle penetrate. My face numbed up as usual and she finished the procedure without any complications that I can tell. The endodontist did not prescribe any anti-biotics and just told me to take some advil for the pain and sent me home.

The anesthesia wore off 15 min after, and the right side of my face was in absolutely excruciating pain! My temple was throbbing and pulsing, my jaw was in agony and I had an intense headache.

(Interestingly enough, several weeks before the same endodontist prepped the same molar for the root canal by using a file on the canals of the tooth the same way, that time i didn't feel anything like this kind of pain! and i was back chewing and using the tooth within 4 days)

I thought this pain was normal I guess after putting in the filling into the canal as that can cause pressure around the tooth, so i decided to give it a couple of days, i was in so much pain (and am now but to a slightly lesser degree) that I couldn't sleep so I've been downing ibuprofen to manage the pain.

By Wednesday the pain was exactly the same and i couldn't take it anymore, i revisited the endodontist and she told me it could be a small infection, antibiotics were prescribed, i also complained about a weird pop inside of my nose when i move my head back and forth (its still there), she said sinusitis and so i was prescribed augmentin.

Its now Saturday and no change at all, Ive been taking amoxicillin, and switched now to augmentin for about a week now (first amox 500 mg from tues to thurs, and now augmentin thurs to sat) and the pain has not gone away! Its less severe now because the injection wound is mostly healed and the tooth inflammation is healing but still pain! a throbbing pain along my cheek, a headache and a throbbing temple! I'm controlling it with ibuprofen 200 mg (which oddly enough people on the internet say doesn't help them with trigeminal pain, but for me it wipes out the pain for several hours but then comes right back)

i also have paraesthesia on the maxillary portion of the Trigeminal nerve, tingling and a weird sensation on my right cheek, under my eyelid sometimes and a slight numbing of the roof of my mouth and upper lip. wth happened to me!!

p.s. I know that trauma to peripheral nerves can heal and im taking all the supplements (b-12, b-6, b-complex, vitamin D) as well as lightly massaging the area and moving my face around multiple times a day to get blood flow to the area to promote healing but this is distressing, i dont want permanent nerve damage and a painful face for life!!!!!!!! as soon as i stop taking ibuprofen the pain comes back!!!

anyone with info please, and i apologize for the length but i have to put in the details so you get a complete picture!

Ahh..

Struggling with same scenario. Taking advil and having weird sensation in my cheek bone and skin. Tingling, feeling of weakness and slightly burning sensation.

What are the things you are doing to help your nerve feel better??

The other thing I have noticed with mine burning sensation is that it goes away when I am not overthinking about consequences of root canal and hurting my nerve.

This week I have been going to Endo for a 3D scan of my tooth, there is still swelling on gum of my root canalled tooth. And oral surgeon has also told me that there are bubbles of infection by the end of root of this tooth. I would rather get it extracted but I am so much worried about nerve issue. However, my tooth is still sensitive to touch and tapping of my dentist.

I'm more concerned about the parathesia that you are reporting rather than the tooth issue. Perhaps a reaction from the local anesthesia. It is rare but I have seen it before. Usual treatment is steroids and goes away most of the time. Ask the OMS.

You mean about burning concern??

Yes, I do feel burning + dull pain on my cheek area and now I do have pressure in my nose too. Hurts while breathing :( I went to oral surgeon but they didnt prescribe me anything. Now next stop is endo. Lets see.

Hey guys just an update to my situation.

I had the root canal done to my upper right molar on Monday July 3, 2017,

Today is Monday July 10, 2017 one full week after the procedure,

the pain I was experiencing on the first day on the right hand side of my face (deep pulsing jaw and temple pain, constant headache) id rate a 10/10. It was terrible and unrelenting.

The pain I was experiencing several days after (about Thursday) id rate 8/10. It was still agonizingly painful, but not enough to drive me over the nearest bridge.

The pain Im experiencing as I write this (Monday 7/10/17 7 days later) is about a 6/10. Painful enough for me to take notice when Im not doing anything, but while walking around, a mild nuisance only.

Its important that I mention that the type of pain hasn't changed, its still the same dull pulsating pain in my jaw, teeth and temple, just to a lesser degree.

Also, the Paresthesia Im experiencing (numbness of my upper right side of my lip and upper right side of my teeth) is still exactly the same, no improvement, although I did feel some tingling in one of the teeth earlier today.


For those suffering through pain, the only way your body is going to heal the nerves is through good sleep. So rest, eat a diet that promotes the healing of the nervous system, and sleep well and deep, taking sleeping pills and pain pills if necessary (always take the lowest dose that does the job, and be careful with sleep mediation).

Another update!


The pain is pretty much non existent now, sometimes it flares up if im stressed but Im sure that this will resolve itself in time as the nerves and the trauma heals.

Im having issues with my sinus now, sneezing most of the time and fluid coming out of my nose as well as a strange popping sound inside of my nose whenever i move my head forcefully back and forth or up and down and sometimes when Im walking ill hear it pop over and over. There is no pain associated with this but it is annoying, I had zero issues with my sinus before so Im sure its as a result of this root canal procedure I had done.

My upper right teeth (where the root canal was done) are still numb as well as my upper right lip but it seems to be getting better. I will be back next week to post on that progress.

My theory on the sinus issues is either it is inflamed or irritated still (its been two weeks since the procedure so this is unlikely) or there is some communication between the filling of the root and the sinus floor of the maxillary sinus thats causing the popping (change in air pressure since the maxillary is an air pocket basically).

Ill be seeing an ENT (Eear Nose Throat) specialist soon to take a CT scan of my sinus and see whats going on.

Other than that I am pain free which is the most important thing one can be, because living with pain is something I wouldn't wish on someone I hated.


Also, I had my original doctor finish the two remaining canals (molars generally have three), and I will tell you this, NO PAIN AT ALL DURING OR AFTER THE PROCEDURE. Everything was done calmly and she took her time and explained everything to me. The doctor that caused me all this pain most likely hit a nerve as well as possibly over filled the canal (which is why I have sinus issues now).

ITS IMPORTANT TO GET A DOCTOR THAT YOU TRUST 1000% AND THAT YOU KNOW BASED OFF THEIR CHARACTER THAT YOU ARE IN GOOD HANDS. DOCTORS THAT ARE DISTRACTED AND RUSH THE PROCEDURE TO GET YOU OUT OF THE DOOR AREN'T WORTH THE MONEY, SPEND MORE EVEN IF IT IS SIGNIFICANTLY MORE AND GET SOMEONE WHO UNDERSTANDS WHAT THEY ARE DOING AND TAKES THEIR TIME.

WHEN YOU AVOID IRREVERSIBLE DAMAGE AT THE HANDS OF A LAZY OR INCOMPETENT DOCTOR, YOU'LL LOVE YOURSELF.
