Tooth erosion and possibly worse

My wife has severe dental phobia. She asked me to take care of everything dealing with her teeth. After two pregnancies with HG each time, moderate to severe erosion is occurring primarily on the inner side of her incisors. She is a mild smoker (about a half pack a day) as well. Why are the inner sides only of her two front central Insiders informally very dark gray while the others appear just eroded and stained.

Any chance of a photo? Most probably some kind of staining. Was she taking iron supplements during the pregnancies?
