Transferring care

I went to a dentist who took an impression for a crown and installed a temporary one. Whilst there, I asked about nitrous, but got no reply. Later, my wife called the dentist, and was told I had been offered nitrous for $250, and had declined. This was a lie.

We checked the insurance, and learned it's available "if necessary". We worked with the insurance to get the approval. The insurance agreed that in my case this is medically necessary. When my wife called the office again to say the nitrous was approved by insurance, she was told that the dental hygienist "didn't want to do it" during a deep-cleaning.

Sorry, but I don't understand why the feelings of a hygienist should override patient care. Either don't offer nitrous at all, or hire staff competent to deal with it.

I have lost all confidence in this dentist. We found another, who said they can do the crown, and the insurance agreed to transfer my treatment to the new provider. I went to the old dentist to sign a release form for my x-rays, and was told that I have no choice but to have them do the crown.

I think it's a Really Bad Idea for them to force me to sit in the chair of a dentist who has proven so untrustworthy. What are my options here?
 
Laugh in their face and go to the new dentist? Retaking a few x-rays isn't a big deal.
