Usual fluoride varnish didn’t help, worried what next?

So I am under public dentist now due to my difficulties with dentist and my usual NHS normal dentist advised to go there because I require a bit more support. This all started in the past when they did a filling and then I kept going back as it didn’t feel right and got myself worked up and developed TMJ.

As part of my plan because of the wear on my teeth from reflux and stress factors I get check ups reguarly, hygenist (3 month and fluoride varnish applied 3 month the alternative is sealant - can’t remember the name now but sure it’s something dura? This time I didn’t make it to 3 month and my usual sensitivity kicked in. In the usual area bottom teeth right, I phoned dentist and they applied fluroide varnish. Since my teeth seem even more sensitivr which seems odd as it’s purpose:// but the area that I went to see about is becoming even more sore. It hurts when I am chewing on food and even after chewing it’s achey.

I had x rays this year and there is no issue showing; just my teeth are worn from above factors. Even the other day at dentist she went my teeth are looking good (ie I’ve kept up brushing and regular check ups and brush with the highest fluroide)
I’m now worrying as the usual fluroide hasn’t helped..what will they do?
I know sealant did help in past but then last time it felt like it was too high then I developed pain in top teeth and a different dentist shaved it down a little which got me worked up :(

I really struggle with the thought of a filling because last time it was wrong. If seleant was applied again is it temp so even if it was high would it come down?
 
I’m unsure if its maybe even the cusp but sure its in the actual tooth! :(
 
Can anyone see what it could be ie a crack or something that would pin point which tooth? I know they look awful (erosion and wear)

If Duraphat didn't work then "Seal & Protect" might work better (This isn't a sealant like you had before despite the name). Could be that the Duraphat got washed off by something too soon to work properly also, so another shot of that might help.

Thanks gordon, is seal and protect stronger? Is it permanent or does it wear down the seal and protect ie if they put too much on?

Last time the public dentist put too much :(
 
It's just a varnish like the Duraphat. Can't really put too much on and it wears off almost immediately.
Tastes bogging though!
