It depends on where you go...some practitioners offer 12-24 month payment plans with no interest, some may offer you a discount if you pay entire amount up front, or you can pay as you go.

Typically no, unless the office you go to has a specific plan that allows you to do so. The payment is dependent on how much the overall amount will be. At the school we have payment plans that range from 6-12 months and the 12 month option breaks the payments down into smaller increments. The practice I'll be working at allows the payment plans to go up to 24 months so you may break them down further.
