A while ago I went to the dentist for a simple cavity filling. In the middle of the procedure the dentist stopped and said he had to drill too much tooth structure and the tooth can no longer support a filling. It will need extracted and either a bridge or implant. This really traumatized me and has me wondering WHY this happened? Is this normal? Is this my fault? A dentist error? Could it haven been avoided? It's really been lingering on my mind

Probably the decay was worse than the dentist expected to find, it happens occasionally, unfortunately working with biological processes isn't an exact science.
If the decay has gone a long way down below the gum line then there's not much hope for the tooth.
