too many questions- i'm doomed :(

hey there, noob here. i don't actually have dental phobia but have other dental operation related phobias and issues. i've an eating disorder and it ruined my teeth big time:( i have 23 teeth left in my mouth:( and almost all of the remaining ones are need to be done.my upper wisdom teeth extracted when i was mid 20s but my lower ones are still there and one of them recently started to hurt big time. all my remaining teeth is falling down peice by peice. i went to see my dentist and she told me they both needed to be extracted and that they can do them both with general anesthesia which freaks me out:( i'm very underweight (5 11/85) and scared that i might not survive the dental work. can't they just remove them with local? i also scared of local anesthetic toxicity but not as much freaky like general. back to my toothache it's episodic comes and goes. sometimes it's bareable sometimes very painful. i tried painkillers like opana, oxy, diclofenac etc they help but when the pain really rages nothing seems to work what to do til i decide? i'm also wondering why wisdom tooth removal after 30 is harder? btw my wisdom teeth aren't impacted but they are half destroyed and i can see dark stuff in them and it's not food?? also both my teeth near my wisdom teeth are extracted, will this make any ease while extraction? i don't want benzos for premedication too would they allow me to go local without it? i also keep purging almost everything i eat and rarely brush my teeth, if i could stop puking and get my teeth done will they ever look normal again?

Yes, it can be done with local alone. Having adjacent teeth out does help.
