Can't numb tooth for root canal part 2

Hi I'm new and looking for some advice please.

A year ago I needed a deep filling on one of my upper premolars which the dentist didn't know needed root canal or not. He put a temporary filling in and told me to come back in a month to assess if it did need rct or not. Anyway he ended up cancelling my appointment and then moved to Spain. I then fell pregnant and in the interest of having a clean pregnancy (I didn't even take paracetamol) opted to wait until after my baby had been born to have any work done.

so this November I booked an appointment with a new dentist who confirmed rct was needed, I underwent part 1 about 2 weeks ago and it was pretty much pain free, nothing I remember. My tooth ached a little for about 2 days after but I assumed it was the nerve killing stuff doing its nerve killing thing.

i returned for part 2 today, had numbing injections and then the temporary filler and packing removed, no problems. The dentist then put his hook thing in (I had my eyes closed) which I very much felt. He did this twice and both times I jumped out of my seat. He then said he couldn't continue as he was hurting me so would have to refer me to a specialist which would cost me £800 because they have anaesthetics he doesn't have. Slightly shellshocked, I looked at him confused for a bit. He then said he could carry on but it would hurt, I said no thanks. We've left it that he's repacked it with the nerve killer stuff again and I'm going back to try again in a week and a half, hoping the nerve will have died this time. If not I go to a specialist. I don't have any day to day pain or problems with the tooth, used to be a little sensitive.

so I have several questions that hopefully someone can answer because I have no clue. Any idea why my tooth isn't dead? Also can something more be done to numb it perhaps? What is the likelihood of the second round of nerve killer working? And if it doesn't is my only option really an £800 bill? I don't have £800. I am except from nhs charges with a maternity card if that makes any difference.

Any advice greatly appreciated as I'm freaking out a little bit.

Since the doctor said that he doesn't have the anesthetics that the specialist has I would say that I am willing to wait the week or two it takes to order a box. The box cost about 50$ USA. It comes with enough to treat many patients at that price too.
