Dentist can’t solve, x ray fine pain chewing

For roughly 2 weeks I have had a pain in my tooth when chewing food or biting down.

Drinks, cold or hot doesn’t affect it/bother it. Yet chewing, it feels like it goes right through the tooth. I am sure it is coming from cusps and I feel it is very back tooth but could be both. My dentist I explained and she isn’t too sure, she carried out an X Ray which everything was fine, the only possible was at the very back she feels there is a wisdom tooth possibly stuck not through.

It’s definitely one of the lower back tooths - I’m 90% its the last one, I’ve always had wear on the teeth which I get fluoride varnish and sealant so often and ths is different to my usual issue of sensitivity when flurodie wears . I hate the way they are
It was happening only on biting but sometimes I am getting a dull achey pain that comes and goes
Along with a shooting pain in head now (it lasts a sec google says ice pick)

I’m worrying because the dentist hasn’t managed to find, when they tapped my tooth nothing really but when I place a cotton bud and bite down, I can feel
I’ve attached photos (please ignore wear colouring) where cotton bud above highest point is one of sore areas

(Is it a crack - but don’t recall biting or having anything, I do eat polos a lot..
Or could this wisdom cause..x ray all good)

It is now reacting a little to drinks too tho different more achey where as chewing definitely still the worst! (Ouch, straight through pain)
 
Sorry, can't diagnose via the internet. Go back to your dentist.
 
@Gordon I have went twice but unfortunately they can’t find the issue. They have said today though they will do a filling, I just hope it will cover the cusp? Can they do that? Also is white filling best or silver - I’m scared of it falling out very anxious over fillings and do wear down a bit! But hate thought of bite being changed🥺🥺
 
@Gordon I know you said can’t diagnose over internet, is that because you can’t see anything obvious?

Ie on other threads 🥺 I’m very anxious as hate thought of filling 🥺
 
No, because there are numerous things that it might be and I can't tell which without looking at you in person. Sorry.

They can't easily cover the cusp with a silver filling, it'd need to be composite. Neither white or silver are "better", it depends on the circumstances which is most appropriate.

You have a lot of tooth substance missing there, it's not going to be a simple job to replace it. Be prepared for a few visits to fine tune things, there's no reason any of them will be painful but there will be a lot of faffing about potentially, unless your dentist gets really lucky :-)
