Have i ruined my life?

So im 18, but because of my ADD I forget to do basic things like eat, shower, and brush my teeth, I haven't found a way to consistently brush my teeth all my life (like I think I've brushed them maybe 200 times in my life) and now I think I have moderate gum disease and im freaking out because last time I had cavity stuff done they didn't numb me enough and I was already pass out scared of dentists before but now, looking at the treatment options for gum disease? I can barely stand before getting queasy from anxiety, im losing sleep and not eating because of this and im not sure what to do- can they just knock me out so Im not aware nor feel anything? Like I sleep and wake up and im on the way to recovery? I still haven't told my parents about my mouth health, planning on telling them latter today, but im terrified they will blame me? Like ik its kinda my fault but ADD is a struggle that they don't really understand, im starting to get thoughts of it would be easier for my mind to just end myself then go get this fixed, even though I haven't had these thought in almost 3 years

@Mathew s Update, my parents didn't believe me at first however I have an appointment today at 11 with a whole new dentist place I've never been to and im not sure how to feel about that? Like is this a bad first impression?

@Braces1973 I'm not sure about the options where you live, but I just had my first appointment in ages today and they just did an initial consult today (x-rays, the dentist looked in my mouth, conversation about my concerns--zero poking or prodding or scraping). Then I will be asleep while they do all the treatment :) It is definitely possible especially if you live near a city. The sedation is going to cost $460 but it is so worth it to me. I hope your initial appointment went well.
