Phobia caused by pain relief not working

I usually have dental work undertaken under sedation. I've tried being referred to receive a general aneasthetic, but was refused. At my last appointment, under sedation, I ended up with injections in the roof of my mouth, which never work and woke up in pain when I was injected at the side of my mouth. My dentist no longer does dentristry work under sedation. I just don't know what to do next ..
