Sedation and high blood pressure

Hi Gordon/Pars,

At my next check up, I'm expecting to be told that I need another filling (having twinges of pain in an upper front tooth and can see some decay, I stupidly didnt tell my dentist when at my last appointment for a new partial :( ) I need to have ALL my treatment done under iv sedation. I am on meds ( ace inhibitors 8mg) trying to control high blood pressure at the moment. Will I still be able to have the deep level of sedation I need? My blood pressure shoots up even under sedation, and taking diazepam beforehand. I know I need to discuss this with my dentist next week, but just thought I'd ask your opinions to put my mind at rest till then.

Has your BP become worse then? ie more uncontrolled? Are you monitoring it at home?

Dental sedation is very safe generally and I'm sure your dentist will be able to help you just fine.

Yes Pars, my bp has become worse since my last sedation appointment. I also had an ecg which showed some heart strain. My meds were increased last week tho. I just worry that I cant have the same deep level of sedation next time. It seems that we phobics worry over every little thing :(
 
Hmm, depending on your BP, your dentist may want it stabilised, realistically. Or refer you to a specialist. But as you are phobic of course your BP will be up when you go to see the dentist. But a letter from your heart specialist may put your wee dentists heart at ease.. (Yes, people, dentists have hearts too!)

I won't do IV on anyone whose BP is higher than 150/100, so if yours is higher than that, I'd want it under control a bit better first.

We measure BP at the start of every session of treatment, it's amazing how many folks are wandering about out there with unknown, uncontrolled, severe hypertension.

I'd certainly feel better about my next iv appointment knowing that my bp was under control and wouldnt cause any problems, especially with the ecg showing some heart strain. At my last dental appointment under iv , it shot up to around 175/105 during treatment.
I'll just hope that the meds help asap, so that I can hopefully finish my treatment (i need another 1 or 2 fillings, top front) I'm seriously thinking about having the four upper front teeth removed and added to my partial and just be done with it! Its difficult enough being so phobic and dealing with having treatment without worrying about my bp too. Waiting between appointments makes things harder to deal with again.
 
175/105 is a bit scary, since IV normally reduces BP by a significant amount. You've got to worry what it would have been without the IV!

This is one of the reasons why IV plus LA is thought to be safer than LA alone btw.
