Chronic Periodontitis - prognosis? cure? what next?

Firstly thank you for your commitment to helping the scared and confused on this marvellous website.

I have had periodontits and severe bone loss diagnosed for about 5 years. I have regular cleanings at my dentist but still have pockets that become painful and inflamed if I don't clean them for a couple of days. I have been told I will be cleaning my teeth with regular visits to the dentist for the rest of my life.

But what I want to know is - is there not something more they can do for me other than just cleaning? Will I always have wobbly teeth? The wobbly teeth do feel uncomfortable. I feel that surely they wont stay in for another 20 years? I am 35 years old. I am wondering will bone/gum grafts help close the pockets? At what point is this type of operation suggested? My dentist is always very serious sounding when he says it is very unusual for someone of my age to have so much bone loss and he would expect to see it insomeone much older.

As my gums are receeding they are becoming more uncomfortable to clean with a brush. Is there even a cure for periodontitis? I would like to have my 2 front teeth straightened - will this ever be possible?

I am not dentist but is your on-going care from a General Dentist or a periodontist?
