Cracked Molar / Teeth Grinding

Hi, really hoping someone can offer some advice!

Early in September I had my lower left molar removed, due to an impacted wisdom tooth (removed in April) causing decay below the gum. The plan was to remove the tooth and have an implant once it was healed.

During the extraction, (not throwing blame around), the 1st molar next to it got cracked, and the dentist managed to hit my top teeth *really* hard while trying to pull the tooth out whole.
I had 2 weeks of dry socket and delayed healing, with an infection as well so it was pretty rough, and because of all that pain, I didn't really notice the cracked tooth causing issues until everything had died down. I went back to the dentist who filed the cracked tooth down, to take some of the pressure off.

It's been 6 weeks now and what I assumed was the healing socket causing pain turns out is the cracked tooth, and I think I also have a cracked upper 1st molar (yet to be confirmed). I've also suddenly started grinding my teeth quite badly, which I've never done before and now have TMJ. I've started physio for that and I have been wearing an amazon mouthguard, but I've also been fitted for a custom one which should be ready next week.

I saw a different dentist last week who said he's reluctant to do anything with the cracked tooth until I've worn the custom nightguard for a while, as he doesn't want to jump into more major tooth work when I've already had 2 teeth removed this year. I'm back again on Friday as the pain isn't going, even with painkillers and I'm actually exhausted after 6 weeks of toothache, so I'm not sure if he will say root canal or crown, or both (I've never had more than a minor filling until this year). The pain comes and goes in such random ways, Sunday was painful all day, Monday was no pain. I can't overly bite down because the tooth has been filed so it doesn't match the top teeth at all & I've not been eating on that side because of the extraction and cracked tooth/teeth. So the TMJ is all on the right and the pain is on the left.

I absolutely cannot do a root canal without some kind of sedation, it's not the pain or the noises or anything, I've got a high pain threshold - it's not being able to physically swallow that sends me into a blind panic and after the mouth guard fitting, I've discovered I have a massive gag reflex too. And I've never been a massive fan of the dentist to start with!

The surgery I'm now at doesn't do any sedation, so I've found another one in London who does offer it, and can do sedation and root canal there.

My (wow, long winded sorry!) question is, if I need 2 root canals, top and bottom on the same side, can they be done at the same time under sedation, and, can I have that treatment there and then go back to my surgery for the crown part? The difference in price is massive so obviously want to save as much as I can.

It's all been going on for so long, I'm just sick of having pain in my mouth and being aware of my teeth all the time. I've lost weight because I can't eat much and I just need it to stop. That's without even thinking about the implant process!

Please can anyone offer me any advice that will help me feel better about everything?

Maybe. You'd need to ask the treating dentist to get a straight answer I'm afraid.
You can certainly get the crown done at your original practice if you want to.
 
@Gordon Thank you - all the not knowing is adding to my already stressed nervous system, I'm back again on Friday so will try to remember all the questions.
 
Write them down and take the list with you. Nobody will mind if you do.
 
@Gordon Thank you. I know it's a normal day in the life for a dentist, but it's a huge thing for some patients & I hate feeling like I'm asking too many questions!
 
If it made you feel better, you could always e-mail your questions in advance? Most dentists quite enjoy having patients actually show some interest in their treatment, it's a bit frustrating when patients just don't seem to care what you're doing to them!
