molar cracked at gumline is sedation necessary

Hi,

I have a upper molar (not wisdom tooth) that had a root canal and broke off at the gumline. It needs to come out. I am deathly afraid of sedation. I had one extraction before with just local it was easy and no pain the tooth was out in 2 min.

I am going to an oral surgon (im in the usa and my general dentist doesn't pull teeth). I have a consultation on wed. Do you think I'll be ok with just local and nitrous/ xanax if needed.

What questions should I ask the surgeon? HElp Im so scared!!
 
I would ask him does he think the extraction is going to be a fairly simple one, or does he think it's going to be more complicated. If he thinks the extraction is going to be fairly simple, then you'll probably be OK with just local anesthesia. If he thinks it's going to complicated (maybe take a bit longer), then maybe you might need nitrous and/or Xanax.

If they have to take it out in pieces (i think section it) is that horrible? do you feel them digging around. I'm really freaking out. Anyone been through this before?
 
I had a lower molar broken at the gumline that needed extracted. The dentist had to break the tooth into 2 halves first, so if you can imagine a molar tooth has 4 roots - he needed to split the tooth so that there were 2 roots in each half. This took seconds and all I felt was pressure. The he removed each half really easily. Again all I felt was pressure. No digging around.
I don't know if the fact that this tooth has had root treatment will make any difference to how easily it can be extracted. Ask the dentist or ask in the Dentistry Questions section.

I had a molar broken down to the gumline extracted last year, and it was easy and painless with local only.

After getting me good and numbed up, the dentist pushed pretty firmly on the tooth from side to side. Each pushing was about fifteen seconds long, and after the fifth one, she said, "it's out!" I never felt anything coming out, or any pain, just the steady pushing. The whole thing took five minutes.

It was a little disconcerting to have someone pushing so hard on my tooth, even though it didn't hurt. And, a couple times a bit of my tooth broke off, which makes a sound that just sounds awful, but again, no pain. My dentist talked very calmly to me the whole time; some music on an MP3 player might be a good idea too.

The whole goal of extraction, I'm told, is to get the surrounding gum tissue to release the tooth, which is does when it feels the tooth being rocked back and forth. That way there's no pulling or tugging, just slowly rocking the tooth back and forth until it falls out on its own.

Also, because it's a pushing, not pulling procedure, there's no need to cut the gums even if the tooth is flush with the gumline. In some cases the dentist might use the drill to make a little shelf on the biting surface of the tooth to give her something to push on. But really, it's quick and painless.

I went through this yesterday.. Dentist gave me the local anesthetic, 2 mins and it was out! There was a bit of pressure but no digging around and it was completely painless..
